  If we do not always get it right, that is no reason to give up or accept a lesser standard. Accepting accidents as inevitable is fatalistic.
  It is important that we make improvements through learning from our own mistakes and from the mistakes of others.

Now here's a thought!

 NOTE 10:      If we learn from our mistakes and from the mistakes of others we won't keep repeating them and the safety of operating processes will improve.
