Accepting a 99.9% or a 1:1000 risk of a fatal accident means that if there are 1000 workers in your plant that either you or one of your workmates has been issued with a death penalty. You won't know which one of you has been chosen or when it will happen until it is too late.

Surely that's not what safety is all about!
